Your First Visit

What to bring

Bringing the right paperwork and imaging to the first appointment usually saves a second visit.

  • A photo ID and your insurance card
  • A list of the medications you currently take, including doses
  • Any imaging you have already had (X-ray, MRI, CT or ultrasound) on a disc or through a patient portal, plus the written reports
  • The name and phone number of your primary care physician
  • Notes from any physical therapy or previous treatment for the same problem
  • Forms your team or employer needs completed
  • Clothing that makes the injured area easy to examine, such as shorts for a knee problem

Records and Imaging

Sending imaging and records

  • Bring the imaging disc itself when you can. Reports alone are useful, but the images are what get reviewed in the room.
  • If your imaging is held at a hospital or radiology group, ask them to release the study to you on a disc or through their patient portal before the visit.
  • Outside operative reports, physical therapy notes and injection records are all worth sending ahead when they exist.

For Referring Physicians

Sending a patient

  • Referring physicians are welcome to call the office directly to discuss a patient before or after a visit.
  • Please send imaging, the radiology report and a brief clinical note with the referral so the visit can be productive from the start.
  • A note summarising the visit and the plan is sent back to the referring physician after each appointment.
